We strive to make your visit to Fresh Coast Family Dental as pleasant and efficient as possible. To save some time at our office, we have provided our patient forms here online. Simply print and fill out these forms before your appointment, then bring them with you for your visit with our experienced dentists in Glendale, California. You will be asked to read and sign the HIPAA form in our office.
